Jay Mason
Chief Executive Officer and CoFounder
MyHealthDIRECT was founded in 2005. As a serial entrepreneur, Jay has a passion for uncovering opportunities that are disruptive innovations and building wildly successful enterprises. He received his Bachelor of Arts degree in business management and marketing from the University of Wisconsin, LaCrosse in 1980. Throughout college he was a Kemper Scholar, one of its first recipients in the country. He worked at CNA Insurance and Principal Financial before starting Professional Insurance Planners of America in 1986, where he achieved one of the top designations as a leading employee benefits agency in Wisconsin. In 1988, Jay launched National Employee Benefits, one of the first chiropractic benefit management firms in the country. Over the next eight years, Jay built NEB to an industry leader serving many managed care plans. Seeing a growing opportunity within complementary benefits, he sought out and led in the merger with American WholeHealth(AWH) in 1998. AWH became the leader in complementary and alternative benefits, with over 30,000 contracted providers and 20 million covered lives. As a venture backed company, investors received a 10x return when the company was sold in 2005. Jay served on the Board of Directors of Congo Initiative from 2006-2009. He is currently serving on the Board of Directors of Concordia University of Wisconsin and on the Board of Directors of the Christian Stewardship Foundation.

Managing Director, Arboretum Ventures
Tim Petersen leverages his broad business experience creating, growing, and financing entrepreneurial firms to target promising capital-efficient investment opportunities. In recent years, he has focused on investments in companies that are changing the way healthcare is delivered. Tim led Arboretum’s investment in HealthMedia (acquired by Johnson & Johnson) and Accuri Cytometers (acquired by Becton, Dickinson and Company) and served on the Board of Directors of each company. He also led Arboretum’s investments in Advanced ICU Care, Inogen, MyHealthDIRECT, and CerviLenz, and presently serves as a director of these companies. Tim is responsible for Arboretum’s financial and fund management activities and is a member of the Investment Committee.
Prior to joining Arboretum in 2002, Tim was the Managing Director of the Zell Lurie Institute for Entrepreneurial Studies at the University of Michigan, leading the creation and rapid growth of the Institute's programs. He also directed the Wolverine Venture Fund (WVF), the Institute's venture capital fund focusing on early-stage life science and technology investments. Previously, Tim spent nine years at the economic consulting firm Industrial Economics, Inc., becoming a member of the firm's senior staff in 1991. Petersen is a member of the Board of Directors of the Michigan Venture Capital Association, where he is slated to become chairman in late 2011. Tim earned a BA in Economics from Williams College. He also holds an MS in Economics from the University of Wisconsin-Madison, and an MBA from the Ross School of Business at the University of Michigan.

David, one of the leading venture capitalists in mid-America, is a well-respected voice in healthcare business and innovation. He founded Chrysalis Ventures in 1993. He focuses on healthcare investing and chairs the management company and funds’ investment committees.
At Chrysalis, David has worked over the years with some of the most exciting growth companies in the Midwest and South, including MyHealthDIRECT, Appriss (sold to Bain Capital), Advanced Academics (sold to Devry), Tritel (merged with Telecorp and sold to AT&T Wireless), Regent Communications (sold to Jacor Communications which was sold to Clear Channel Communications), ActaMed (merged with Healtheon and then with WebMD), Tech Republic (sold to Gartner Group) and High Speed Access Corp (IPO and later sold to Charter Communications). In addition, David has served on the Board of Directors of Humana Inc. (NYSE: HUM) since 1993, serving as Vice Chairman from 1996 to 2005 and as Chairman from 2005 to 2010. Jones chairs the board of Business Leaders for Education and serves on the Executive Committee of the Business-Higher Education Forum. Currently, David is a director of the Humana Forum, Inc. and is active on the grants committee of a private foundation. He is also a Board member of the National Committee on U.S.-China Relations. David has a BA from Yale University and a JD from Yale University Law School.

Cobb, as an Entrepreneur-in Residence evaluates new business opportunities and assists current portfolio companies in the technology sector. Doug is a board member to Chrysalis portfolio companies, MyHealthDIRECT and Regent Education.
A lifelong entrepreneur, Doug has a diverse background in media and technology. Most recently, from 2000—2010, he was Chairman and CEO of Chrysalis portfolio company Appriss, Inc., a Louisville-based company that provides innovative information technology solutions to assist law enforcement efforts and improve the efficiency and effectiveness of criminal justice agencies. Previously, from 1997 until 2000, Doug served as the founding President and CEO of Greater Louisville Inc., Louisville’s leading economic development organization. Before joining Greater Louisville Inc., Doug was a Managing Director of Chrysalis from 1993 – 1997. Prior to co-founding Chrysalis, he was the Founder and CEO of The Cobb Group, the world’s leading publisher of newsletters for personal computer users, which was successfully sold to publisher Ziff-Davis. He is a three-time Inc. 500 CEO, a three-time Kentucky Entrepreneur of the Year and a member of the Junior Achievement Kentucky/Southern Indiana Business Hall of Fame. Doug holds a B.A. from Williams College, magna cum laude, and an M.S. in Accounting from the New York University Graduate School of Business Administration.

As Chief Medical Officer of Applied Pathways and over the course of his career, Dr. Woodburn has focused on providing strategic guidance to accelerate quality improvement and consumer-centered systems change.
Dr. Woodburn previously held the position of Vice President of Clinical Initiatives for United Healthcare and Optum Health, where he was a leader of the TeleMedicine ConnectedCare enterprise. He has served as an Entrepreneur-in-Residence for Lemhi Ventures and as Chief Medical Officer for MinuteClinic, where he helped to lead the national expansion of retail clinics and acquisition by CVS Pharmacy. Dr. Woodburn also spent over a decade at Blue Cross Blue Shield of Minnesota, where he was a founder and leader of the Blue Cross Blue Shield Association’s Medical Management Forum. He is a residency and fellowship trained Emergency Medicine physician and holds two engineering degrees from the University of Wisconsin-Madison.

Mike offers more than 25 years experience effecting change in the healthcare industry and national policy and politics. He is known for his ability to identify and execute growth opportunities and accelerate healthcare marketing, business development and innovation efforts. Mike is responsible for Navvis and Company’s brand and marketing portfolio. He is the Managing Director of Brand+Lever, Navvis’ joint venture with BVK to develop and execute integrated brand and marketing strategies to grow high-performing clinical enterprises.
Prior to joining Navvis Mike held executive positions in Milwaukee with Wheaton Franciscan Healthcare and Aurora Health Care. Before entering the healthcare arena Mike held leadership positions for members of Congress, serving as legislative director for U.S. Congressmen Bill Emerson and Henry Hyde in the 1980’s and as chief of staff for then Wisconsin State Senator Margaret Farrow in the mid-1990’s. A former lobbyist, he remains connected and conversant on the implementation of health policy reforms at the state and national levels. He belongs to and is active in several professional societies and healthcare organizations. He has served on numerous boards including the Wisconsin Hospital Association, the Wisconsin Medical Group Management Association; the Institute for Illinois; and boards for a number of community non-profits. Mike graduated from the University of Wisconsin-Madison with honors in Economics and History.

Jacque J. Sokolov, M.D., is chairman and chief executive officer of SSB Solutions, Inc. a national healthcare management, development and investment firm. Dr. Sokolov, currently serves as a director of Hospira (NYSE:HSP), MedCath Corporation (NASDAQ:MDTH), the National Health Foundation (NHF), Phoenix Children’s Hospital, and the White House Health Project. He previously served as the vice president of healthcare for Southern California Edison Corporation (NYSE:EIX) and chairman of the board of directors for Coastal Physician Group, Inc. (NYSE:ERDR), as a director of the American College of Medical Quality (ACMQ) and the National Business Group on Health (formerly WBGH). Dr. Sokolov received his BA in medicine from the University of Southern California and his M.D. from the USC School of Medicine. He completed his internal medicine residency at the Mayo Graduate School of Medicine and his fellowship in cardiovascular diseases from the University of Texas-Southwestern Medical School. He previously held and currently holds academic appointments and advisory board responsibilities in the Schools of Medicine, Business and Public Health at Harvard, the Massachusetts Institute of Technology, the University of Pennsylvania, the University of California, Los Angeles, and the University of Southern California.
